Eligibility Requirements

To qualify for coverage, you must meet all of the following criteria:

Be a Farm Bureau member.
Be a full-time Michigan resident and live in a Blue Care Network HMO service area.
You do not have Medicaid, group or any other supplemental health care coverage.
Have not had prior Farm Bureau sponsored BCN health coverage that canceled within the past year.
Have Medicare Parts A and B.
