About the premium rates
Rates represent the cost of one month of coverage. Members are billed quarterly in February, May, August and November. These rates are good until August 20, 2012.
Family continuation rider (F-Rider) rates
F-Rider refers to a family continuation rider; a child who is over the age of 19 but still a dependent of their parents. To qualify as an F-rider, a dependent must be:
- Unmarried.
- Between the ages of 19 and 25.
- Dependent on you for more than half of his/her support.
- A member of your household.
- Related to you by blood, marriage or adoption.
- A full-time student for at least five months of the year OR had a gross income of less than four times the personal exemption amount identified by the IRS Gross Income Test.
The F-rider rate is an additional charge that is added on to the other family members' rate.
