How To Apply For Coverage

To apply for coverage in the Blue Care Network HMO plan, call or visit your local county Farm Bureau administrative manager.

For the phone number and location of the office nearest you, select your county from the dropdown menu below and press "View County".

The Blue Care Network HMO plan is guaranteed issue; no medical underwriting is required. There is no preexisting condition waiting period or maternity waiting period on the Blue Care Network HMO plan.

Effective date of coverage

Completed applications for the Blue Care Network HMO plan that are received at Michigan Farm Bureau by the 10th of a month will be effective the 20th of the following month.

This is a prepaid health coverage plan. Claims will not be paid and acknowledgement of coverage will not be given until the first premium payment is made.
