Our Blue Care Network HMO is a comprehensive plan with extensive preventative and wellness benefits.
With our Blue Care Network HMO plan, office visits are just a $20 copayment per visit. And there is no deductible to meet on this plan before services are payable. Covered services - including inpatient and outpatient hospital, surgical, preventative and wellness care - are paid in full with some minor copayments required, when authorized by your Blue Care Network primary care physician. Prescription drug coverage is automatically included on this plan with just a $10 copayment for generic drugs and a $40 copayment for brand name drugs - or 50%, whichever is less - per prescription or refill.
As a member of our Blue Care Network HMO plan, you'll choose a primary care physician for each family member from our extensive directory of physicians. Your primary care physician is then responsible for coordinating all the health care needs of you and your family. By seeing one primary care physician on a regular basis, your physician can become more familiar with your health care needs and keep you healthy through preventative services and early disease detection.
Blue Care Network's Woman's Choice Program allows all female members the opportunity to directly access an affiliated OB-GYN without a referral from their primary care physician for all obstetrical care and routine nonsurgical care. Children under the age of 18 can take advantage of the Pediatric Choice Program to see an affiliated pediatrician without a referral from their primary care physician.
Blue Care Network has more than 10,000 primary care physicians and specialists and contracts with 110 hospitals throughout Michigan. Services must be provided or arranged by the member's primary health care physician or health plan. Find a Blue Care Network Primary Care Physician.
