Comprehensive Major Medical


The Comprehensive Major Medical (CMM) plan is a traditional, comprehensive plan covering everything from inpatient care to outpatient care, laboratory tests and x-rays to diagnostic office visits. The advantage of a traditional plan is you can use any physicians you choose with no restrictions. However, if you use Blue Cross Blue Shield of Michigan (BCBSM) participating physicians, you'll minimize your out-of-pocket costs. Approximately 90% of the physicians and all acute care hospitals in Michigan participate with BCBSM and our CMM plan. Participating physicians accept the BCBSM approved amount as payment in full for covered services. This means you only pay any deductible and copayments required by your contract, and non-covered services.

Our CMM plan is available in four deductible options: $250, $500, $1,000 and $1,500. You only need to meet two deductibles per family. Once your deductible is met, BCBSM pays 80% of your covered benefits for general services and you pay the remaining 20%. The maximum you'll pay in copayments for general services is $1,000 per calendar year. At that point, BCBSM will pay 100% of your covered benefits for general services for the remainder of that calendar year.
